MOTUS Registration Form

FMCSA has introduced a new system called MOTUS. Companies with an active USDOT number are now required to create and maintain a MOTUS account.

Our office offers assistance with creating and registering your FMCSA account for use with the MOTUS system for a fee of $150 per company. Please note that the account owner/company representative must be available by phone during the registration process because FMCSA requires:

• Facial recognition verification
• A picture of a government-issued ID
• Identity confirmation through Login.gov
If there are issues with your login credentials and we must complete an account reset before registering your company in MOTUS, the total fee will be $185 due to the additional verification and recovery steps required.

In addition to registering for a MOTUS account, FMCSA requires users to log in to their FMCSA portal at least once every 90 days to maintain account access and avoid potential login issues. We offer an optional account maintenance service for customers who would like our office to manage these required logins.

FMCSA Account 90-Day Login Maintenance Agreement
This FMCSA Account 90-Day Login Maintenance Agreement ("Agreement") is entered into by and between All About Trucks, Inc. ("Service Provider") and __________________ ("Client").

1. Authorization
Client hereby authorizes All About Trucks to access and log into the Client`s FMCSA account solely for the purpose of performing the required maintenance login needed to help maintain account access. This service does not include filing updates or making changes to the account unless separately authorized.

2. Scope of Service
All About Trucks is authorized to log into the Client's FMCSA account once within each ninety (90) day period to help maintain account access in accordance with FMCSA requirements. For the avoidance of doubt, All About Trucks will perform the scheduled login service within each ninety (90) day interval regardless of any updates, filings, or account activity performed by the Client or any third parties.

3. Ongoing Permission
By signing this Agreement, Client grants ongoing authorization for All About Trucks to perform the above service without requiring additional consent prior to each login. This authorization will remain in effect until:
(a) Client provides notice to terminate the service, or
(b) The service is no longer necessary or relevant due to changes in FMCSA requirements or systems.

4. Communication Acknowledgment
Client understands and agrees that All About Trucks will not contact Client prior to each 90-day login and will perform the service automatically under this Agreement, regardless of any updates or account activity performed within that time frame.

5. Fees
Client agrees to pay a service fee of $40 per login, which will be charged each time All About Trucks performs the authorized login service.

Optional addition:
Invoices are due upon receipt unless otherwise agreed in writing.

6. Client Responsibilities
Client agrees to provide accurate and current FMCSA account login credentials (including Login.gov credentials, if applicable) and to promptly notify All About Trucks of any changes to account access information.

7. Limitation of Responsibility
All About Trucks will take reasonable measures to maintain the confidentiality of Client credentials and perform services in good faith. However, All About Trucks is not responsible for system outages,
changes in FMCSA policies, Login.gov requirements, or other government systems, or issues beyond its control that may affect account status.

8. Termination
Client may terminate this Agreement at any time by providing written notice, including by email. Any services performed prior to termination remain payable.
